NEW PATIENT INFORMATION
To make certain you receive the time and attention required to provide
optimal care, we ask that new patients arrive 20-30 minutes prior to their first
appointment to complete the necessary registration process. Please bring the
following to your initial appointment with Virginia Cancer Institute:
- Insurance Card - please bring to each appointment
- All medications you are currently taking either an accurate list or the
medications themselves
- Completed pre-registration forms, if available
- Insurance Referrals, if required
OFFICE HOURS AND SCHEDULING
Patients are seen by appointment only. To schedule an appointment, please call
your physician's primary office location, Monday through Friday, during office
hours.
If you are going to be late for an appointment, or if re-scheduling is
necessary, please notify us as soon as possible. The physicians
of Virginia Cancer Institute conscientiously attempt to examine patients at their designated appointment
time. However, due to the nature of our practice, occasionally scheduling
delays occur. If we do experience a schedule delay, we ask for your
understanding and courtesy.
In the event of inclement weather, Virginia Cancer Institute will keep
our offices open as long as the safety of our patients and staff is not at
risk. Please call your Virginia Cancer Institute office, our answering service will advise you of
any delays.
EMERGENCIES If at any time you experience a life-threatening emergency, call 911
immediately.
In the event of a non-life-threatening medical emergency during office hours,
call the office where you are generally examined. Please identify the call as
urgent or as a non-life-threatening emergency, so that one of our nurses or
physicians can assess your situation and provide you with instructions.
If a non-life-threatening medical emergency occurs after office hours, please
call the office where you are generally examined. Your call will automatically be forwarded to our medical
answering service, who will contact our on-call physician. A member of the
Virginia Cancer Institute medical staff is available 24 hours a day,
seven days a week. If your call is not returned within 30 minutes, please
place a second call.
PRESCRIPTION POLICIES
To assure your safety and well-being, the physicians of Virginia Cancer
Institute prefer to provide written
prescriptions to you at the time of your office visit. Although your
physician may be willing to call in your prescription, this is not the
procedure of choice, and, by law, certain medications cannot be obtained
without a written prescription. Please call your pharmacist to request a
refill on your prescription, or ask your pharmacist to call us with the
request for a refill order.
Due to the nature of your medical condition, it is very important that you not
run out of your medications. Be sure to call for a refill, at least two days
prior to depleting your supply of medication.
LABORATORY TESTS Most laboratory tests are performed in our offices. Some specialized tests,
however, require that we send the samples to a hospital or private laboratory.
A separate bill will be sent to you by the hospital or laboratory for these
procedures. Please be certain that you have your insurance information with
you each time a laboratory test is performed. If you receive a statement from
a hospital or private laboratory, and there is no indication that your
insurance carrier has been notified, please call their business office to
provide them with your insurance information, so that they can submit a
statement of charges to your insurer.
HOSPITALIZATION
The physicians of Virginia Cancer Institute admit patients to most of the hospitals in the Richmond area;
however, not every physician practices at every hospital. If hospitalization
is required, to maintain the continuity of your care, we prefer that you stay
in a hospital attended by your physician. If you prefer to be admitted to
another hospital, one of our other physicians who practices at that facility
will oversee your care during your hospitalization. The staff of Virginia
Cancer Institute will make the
necessary arrangements for your hospital admission. It is your responsibility,
however, to obtain any required approvals for hospital admission from your
insurance carrier.
MEDICAL RECORDS
Your medical record is confidential and will not be given to anyone unless
requested in writing by you. To request copies of your records, please call
your physician's primary office. Our Virginia Cancer Institute staff will be glad to fax your medical
records to another physician, or copy them for you for a small administrative
fee.
